Dental Reminder For This Year

Dental Examination forms are due May 15, 2018 for current Kindergarten and 2nd grade students. Reminders were sent home in February to the students who have not turned in their forms yet. Please contact the health office if you need another form.

New State of Illinois Vaccination Requirements

All students entering Kindergarten must have evidence of two varicella vaccinations and two MMR vaccinations prior to the first day of school.  

All students entering 6th grade must have evidence of a completed Hepatitis B vaccination series, a Tdap booster, two varicella vaccinations, two MMR vaccinations, and a dose of the meningococcal conjugate vaccine.

Health Information for 5th Grade students

A health packet will be sent out in the next couple of weeks for students that will be in 6th grade next year. This packet will contain a copy of mandatory physical exam information, dental exam information and interscholastic athletics requirements.

All of the proper health forms are also available at the Brentwood Health Office. If you have any questions please call (847) 472-3445. Any completed physical exams may be turned in to the Brentwood Health Office before the end of the year. They will be forwarded to the Junior High. A completed physical exam must be turned in before students can compete in sports at the Junior High.

Staying Healthy at School

Spring is a season of change. The mornings are chilly and then warm up. Please make sure your child is adequately dressed to go outdoors for recess with a jacket. Dressing in layers is a good idea. Spring colds and allergies can also affect your child. Please check your child before school. School District 59 policy states “children cannot come to school if they have a fever greater than 100 degrees. They must be fever free for 24 hours without the use of medication before they can return to school. Children should also stay home if they have vomiting, diarrhea, rash, excessive coughing, inflamed or reddened eyes, sore throat, or an earache.” When your child comes to school he or she is expected to be able to participate in all activities including gym and recess. A doctor’s note is required to exclude your child from activities for 2 or more days. A signed parent note with reason for activity exclusion is acceptable for one day. If your child needs medication at school (including “over the counter” medication), a Medication Authorization Form must be completed by your doctor. Parents must bring the medication to school. Medication must be in its original container and labeled with the child’s name.

Updated Emergency Contact Information

It is extremely important that we have updated personal as well as emergency contact phone numbers.  It has been difficult to reach many parents during the school year due to non-working or disconnected numbers.  Please be diligent in providing the office with the most current phone numbers as soon as possible. This includes immediate family as well as anyone listed as an emergency contacts.  Make sure the people that you list as an emergency contact are also current and easily reachable. They should be available to pick up your child in your absence if you are ill. For families that may have issues with their phones, please provide the office with the phone number of a responsible family member, friend or neighbor who will be reachable when you are not. There have been instances where we go through all the numbers listed as emergency contacts and they are all disconnected or we can’t leave a message.
